How to File a Request
The Illinois Freedom of Information Act, (FOIA) requires that all public bodies make public records available to the public, subject to certain exemptions. It is the policy of the Streamwood Park District to fill all requests for public records in as timely a fashion as possible. By law, the maximum time to produce the information for routine requests is five business days; commercial requests are 21 business days. In rare cases, you will be notified, in writing, if an extension is needed, as allowed by the Act.

A FOIA request must be made in writing and should reasonably describe the records you seek. There is no specific form required, but being clear and specific helps ensure your request is processed efficiently.
Making a FOIA Request in Writing
To submit a Freedom of Information Act (FOIA) request, it is essential to follow specific guidelines to ensure your request is processed effectively.
- Written Format: Your request must be in writing. This can be done via:
- Fax
- Traditional mail
- Reasonable Description: Clearly describe the records you are seeking. This helps the agency locate the information efficiently.
- Contact Information: Include your:
- Name
- Preferred telephone number(s)
- Mailing address
- Email address (optional)
Example of a FOIA Request:
Subject Line: FOIA Request for [Specific Records]
Request Statement: “I am requesting copies of [specific records] related to [subject].”
Reasonable Description: These records should include [details about the records, such as dates, types, etc.].”
Contact Information: [Your Name], [Your Address] or [Your Email].”
Additional Tips
- Be Specific: The more specific you are, the better the chances of a timely response.
- No Standard Form Required: There is no specific form you must use, but clarity is crucial.
- Avoid Questions: FOIA is designed for records, not to answer questions.
